To update SR/SP address: Navigate to the Member Address under Contacts

  1. Verify any address before updating the new address. If SR or SP cannot, ask them something else such as the city of their birth, Mother’s name, Mother’s maiden name, etc.
  2. Identify the new address as the “valid” address in AZCares. This action will automatically change any addresses previously coded as “Valid” to “Historic”.

Click Pencil Icon under Actions for the address that needs to be updated

  1. Member Address Screen pop up will populate your screen

Click on the Add/Update Address button

  1. Address Details Screen pop up will populate your screen
  2. Update the already filled in address and click on Validate.
  3. Select Address pop up will appear. Select the radio button for either the “Use Standardized Address” or “Use Entered Address” as appropriate. Click Ok.
  4. Return to the Member Address screen, update the Verification Date to today’s date. Click Save.

To enter new address for SR/SP:

Navigate to the Member Address under Contacts

  1. Select “New”
  2. Populate all required/necessary fields
  3. Address Type
  4. Address Status
  5. Verification Source
  6. Verification Date
  7. Select “Add/Update Address”
  8. Populate necessary fields with the new/updated address.
  9. Select “Validate”
  10. Select Address pop up will appear. Select the radio button for either the “Use Standardized Address” or “Use Entered Address” as appropriate.
  11. Click “OK”
  12. Click “Save”
  13. NOTE: The steps listed above need to be repeated if you’re adding more than one address i.e. residential and mailing.

Document:

Navigate to Activity History under Case

  1. Click on New. Document the new address, as well as any updates to the customer’s phone number(s) and/email address, in your Activity History comment.

Historical Address is needing to be updated as the Valid address:

Navigate to the Member Address under Contacts

  1. Click Edit Details (pencil of) the Historic address
  2. Update Address Status to “Valid”
  3. Ensure Verification Source is on the correct party SR or SP
  4. Update the Verification Date to today’s date.
  5. Click “Save”

Document:

Navigate to Activity History under Case

  1. Document the address change as well as any updates to the customer’s phone number(s) and/email address, in your Activity History comment.
